What the evidence says about creatine and thinking

The trials are small, short and mostly about muscle. What survives when you read only the cognitive ones is narrower than the internet suggests — and more interesting.

What the two meta-analyses found

Prokopidis and colleagues pooled the randomised trials of creatine and memory in 2023 and reported a benefit; a later re-analysis narrowed that effect to older adults, roughly 66 to 76. That narrowing is the most useful sentence in the whole literature, because it is the difference between a finding and a finding about you.

Xu and colleagues pooled 16 randomised trials in 2024 — 492 participants across healthy and clinical groups — and reported signals on memory, attention time and processing speed. The trials are small, the populations are mixed, and the paper carries a published corrigendum and commentary. A pooled estimate inherits the weaknesses of what it pools; it does not launder them.

Put the two together and the shape is consistent: the clearest effects appear where the brain's own creatine supply is likely to be under strain, and they fade where it is not.

Why the strain conditions matter more than the dose

Three groups keep recurring in the trials that report an effect: older adults, vegetarians and vegans, and people who have been kept awake. All three share one feature — a lower or more heavily drawn-on pool to begin with. That is what makes the pattern worth taking seriously rather than dismissing as noise: the effect is not everywhere, it is somewhere specific, and the somewhere is predictable in advance.

It also sets the honest expectation for everyone else. If you sleep adequately and eat meat or fish, you are the population in which the effect is smallest and least consistently found. That is not an argument against taking it. It is an argument against expecting to notice anything.

What the trials measured, and what they did not

Every cognitive number in this literature comes from a laboratory task: recall a list, respond to a cue, hold a sequence. Those tasks are chosen because they are stable and repeatable, not because they resemble a Tuesday afternoon. No trial in this set measured whether anyone wrote a better report, studied more effectively, or felt clearer.

So the correct summary of a positive result is: on this task, in this group, over these weeks, the supplemented group scored better than the placebo group. Anything beyond that sentence is added by the person quoting it.

Safety, briefly and without softening

The International Society of Sports Nutrition's 2017 position stand reviewed the safety literature and found no evidence of harm from creatine monohydrate at recommended doses in healthy people. Reported effects are water-weight gain and gastrointestinal upset, the latter mostly at high loading doses.
